The pollution in the Tijuana River has resulted in environmental and public health concerns on both sides of the border, including beach closures, contamination of water supplies, and harm to aquatic ecosystems and wildlife. Efforts to address pollution in the Tijuana River include improved wastewater treatment infrastructure, cross-border cooperation on environmental management, pollution monitoring and assessment, and public education and outreach on waste management and pollution prevention.

The most common symptoms of recreational water illness (i.e. getting in the water) are diarrhea, skin rashes, ear pain, cough or congestion and eye pain.
